Seven out of 10 deaths each year occur as a result of chronic diseases. With a rising older adult population comes greater risk of chronic diseases, such as arthritis, heart disease, and diabetes. The Centers for Disease Control and Prevention (CDC) seeks to foster local health programs aimed at reducing chronic illnesses by offering Community Transformation Grants (CTG). Local leaders seeking to implement new initiatives to reduce chronic diseases in their area should consider applying for these grants as a way to fund those initiatives.Key Points
Over $100 million in prevention funding has already been awarded.The website provides three links, including funded states and communities, CTG program descriptions, and CTG resources. By visiting the program descriptions link first, the local leader can evaluate those states and communities who were previously awarded grants. A particular emphasis is the Small Communities Program, which will grant $70 million to communities of 500,000 residents or less. Applicants seeking to implement evidence-based CTG initiatives will be awarded one-time grants for a two year period of time. However, grants are awarded across multiple industries including housing, transportation, education, and public health.How to Use
