Seven out of 10 deaths each year occur as a result of chronic diseases. With a rising older adult population comes greater risk of chronic diseases, such as arthritis, heart disease, and diabetes. The Centers for Disease Control and Prevention (CDC) seeks to foster local health programs aimed at reducing chronic illnesses by offering Community Transformation Grants (CTG). Local leaders seeking to implement new initiatives to reduce chronic diseases in their area should consider applying for these grants as a way to fund those initiatives.
