Overview

Person-centered planning generally refers to considering older adults or disabled persons’ needs, goals, preferences, cultural traditions, family situation, and values. At the center of person-centered planning is the focus person, close friends and family members, and the caregiver or other authority of the focus person’s life direction. This manual, organized by the Institute for Community Integrationat the University of Minnesota, is a guide for person-centered planning facilitators.
