True Vault Hunter Mode is the hardest difficulty level for Borderlands 3 and, unlike Mayhem Mode, if you want to play through the game at this level of difficulty, you'll have to start right back at the beginning of the plot. Thankfully, you can do this with your existing Vault Hunter, keeping all the loot you've earned, their current level and Guardian Rank.
