إذا ظهرت عليك أعراض مرض فيروس كورونا المستجد 2019 (كوفيد 19)، أو تعرضت لفيروس كوفيد 19، فاتصل بالطبيب. وأخبر الطبيب أيضًا بما إذا كنت قد خالطتَ أي شخص ثبتت إصابته بفيروس كوفيد 19. قد تختلف العوامل المستخدمة لتحديد ضرورة إجراء الفحص للكشف عن الفيروس المسبب لمرض كوفيد 19 باختلاف محل الإقامة. وبناءً على المكان الذي تقيم فيه، تتحدد إمكانية التوجه إلى العيادة لتحديد ما إن كان الفحص مناسبًا ومتوفرًا. ففي الولايات المتحدة، يحدد الطبيب ما إذا كان سيجري فحوصات الكشف عن الفيروس المسبب لمرض كوفيد 19 بناءً على مؤشرات المرض وأعراضه، وما إذا كنت قد خالطتَ شخصًا ثبُتت إصابته بفيروس كوفيد 19. وقد يرجح الطبيب أيضًا إجراء الفحص إذا كنت معرضًا بنسبة كبيرة للإصابة بمضاعفات شديدة، أو إذا كنت مُقدِمًا على الخضوع لإجراء طبي. فإذا كنت قد خالطت أحد المصابين بفيروس كوفيد 19 مخالطة لصيقة، لكنك أُصبت بفيروس كوفيد 19 خلال الأشهر الثلاثة الأخيرة، فأنت لست في حاجة إلى الفحص. وإذا كنت قد تلقيت جرعات اللقاح كاملة وكنت على مخالطة لصيقة بشخص مصاب بفيروس كوفيد 19، فعليك إجراء الاختبار بعد مدة تتراوح بين 5 و7 أيام من مخالطته. لفحص الإصابة بفيروس كوفيد 19، يأخذ مزود الرعاية الصحية عينة من الأنف (مسحة بلعومية أنفية)، أو من الحلق (مسحة حلقية)، أو من اللعاب. ثم تُرسَل العينات إلى المختبر لتحليلها. وإذا كان سعالك مصحوبًا ببلغم، فقد تُرسل عينة من البلغم كذلك إلى المختبر لفحصها. وقد اعتمدت إدارة الغذاء والدواء الأمريكية فحوصات منزلية للكشف عن فيروس كوفيد 19. وتتوفر تلك الفحوصات فقط بوصفة من الطبيب.

العلاج

في الوقت الحالي، حصل دواء واحد فقط على الموافقة لعلاج مرض كوفيد 19. ولا يوجد علاج شافٍ متاح لمرض كوفيد- 19. والمضادات الحيوية ليست فعالة ضد العدوى الفيروسية مثل كوفيد- 19. ويُجري الباحثون اختبارات على العديد من العلاجات المحتملة. اعتمدت إدارة الغذاء والدواء الأمريكية عقار ريميديسيفير (Veklury) المضاد للفيروسات لعلاج كوفيد 19 لدى البالغين الذين أدخلوا إلى المستشفيات، والأطفال من عمر 12 عامًا فأكثر الذين يخضعون للعلاج في المستشفى. ومن الممكن أن يُوصف ريمديسيفير للمرضى الذين يدخلون المستشفى بسبب إصابتهم بمرض كوفيد 19 ممن يحتاجون إلى أكسجين تكميلي أو المرضى الأكثر عرضة للإصابة بمضاعفات خطيرة. ويُعطى عن طريق الحقن في الجلد (من خلال الوريد). اعتمدت إدارة الغذاء والدواء الأمريكية دواءً يُسمى Paxlovid يحتوي على عقار نيرماترلفير، وهو دواء يمنع نشاط إنزيم معين يحتاجه الفيروس المسبب لمرض كوفيد 19 في عملية التكاثر، بالإضافة إلى عقار مضاد للفيروسات يُسمى ريتونافير يساعد على إبطاء تحلل عقار نيرماترلفير. واُعتمد دواء Paxlovid لعلاج حالات كوفيد 19 الخفيفة إلى المتوسطة لدى الأشخاص بعمر 12 عامًا أو أكبر الذين يكونون أكثر عرضة للإصابة بأمراض خطيرة. وتُعطى هذه الأدوية عن طريق الفم على شكل أقراص. اعتمدت إدارة الغذاء والدواء الأمريكية دواءً آخر يُسمى مولنوبيرافير لعلاج حالات كوفيد 19 الخفيفة والمتوسطة لدى البالغين المعرضين بشكل أكبر للإصابة بأمراض خطيرة ولا يستطيعون اللجوء إلى خيارات العلاج الأخرى. ويُعطى هذا الدواء عن طريق الفم على شكل أقراص. اعتمدت إدارة الغذاء والدواء الأمريكية دواء التهاب المفاصل الروماتويدي باريسيتينيب (Olumiant) لعلاج كوفيد 19 في بعض الحالات. ويأتي دواء باريسيتينيب على شكل أقراص، ويبدو أنه يقاوم كوفيد 19 عن طريق تقليل الالتهاب، بالإضافة إلى مفعوله المضاد للفيروسات. يمكن استخدام دواء باريسيتينيب في علاج المرضى الذين يدخلون المستشفى بسبب الإصابة بفيروس كوفيد 19 ممن يوضعون على أجهزة التنفس الاصطناعي، أو من يحتاجون إلى أكسجين تكميلي. ومن أمثلة أدوية الأجسام المضادة أحادية النسيلة العقاقير سوتروفيماب وبيبتيلوفيماب، والمركب الذي يجمع بين العقارين باملانيفيماب وإتيسيفيماب، والمركب الذي يجمع بين العقارين كاسيريفيماب وإيمديفيماب. ولم تثبت فعالية بعض أدوية الأجسام المضادة أحادية النسيلة، مثل باملانيفيماب وإتيسيفيماب وكاسيريفيماب وإيمديفيماب، في علاج حالات كوفيد 19 المصابة بمتحور أوميكرون. ولكن رغم ذلك، يُستخدم كل من سوتروفيماب وبيبتيلوفيماب في معالجة حالات كوفيد 19 التي يسببها متحور أوميكرون. تُستخدم هذه العقاقير لعلاج حالات كوفيد 19 الخفيفة والمتوسطة لدى الأشخاص الأكثر عرضةً للإصابة بمضاعفات خطيرة بسبب عدوى كوفيد 19. ويتوفر هذا العلاج على هيئة جرعة واحدة تحقن في الذراع (من خلال الوريد) في إحدى العيادات الخارجية. وكي تحقق هذه الأدوية أعلى فعالية لها، ينبغي تلقيها فور بدء ظهور أعراض كوفيد 19 وقبل الوصول إلى مرحلة دخول المستشفى. وقد أوصت معاهد الصحة الوطنية الأمريكية باستخدام الكورتيكوستيرويد ديكساميثازون للمقيمين في المستشفى والمصابين بالحالات الحادة من كوفيد-19 ممن يحتاجون إلى أكسجين إضافي أو استخدام جهاز التنفس الصناعي الميكانيكي. ويمكن استخدام أدوية الكورتيكوستيرويدات الأخرى، مثل البريدنيزون أو الميثيل البريدنيزولون أو الهيدروكورتيزون، في حالة عدم توفر الديكساميثازون. في بعض الحالات، قد يُعطى دواء توسيليزوماب أو باريسيتينيب مع ديكساميثازون للمرضى الذين يدخلون المستشفى ويُوضعون على جهاز التنفس الصناعي أو يحتاجون إلى أكسجين تكميلي. منحت إدارة الغذاء والدواء الأمريكية أيضًا تصريحًا باستخدام العلاج ببلازما النقاهة ذات المستويات العالية من الأجسام المضادة لعلاج كوفيد 19. وبلازما النقاهة هي دم يتبرع به الأشخاص الذين تعافوا من كوفيد 19. وبلازما النقاهة التي تحتوي على أجسام مضادة عالية يمكن استخدامها لعلاج بعض الأشخاص المقيمين في المستشفى والمصابين بمرض كوفيد 19 في مراحله الأولى أو يشكون من ضعف الجهاز المناعي. قد يُصاب كثيرون بمرض كوفيد 19 بأعراض خفيفة ويمكن معالجتهم بالرعاية الداعمة. تهدف الرعاية الداعمة إلى تخفيف الأعراض، وقد تشمل: تناول مسكنات الألم (الأيبوبروفين أو الأسِيتامينُوفين) تناول شراب أو دواء للسعال الراحة تناوُل السوائل ولا يوجد دليل على أنه يلزم تجنب تناول الأيبوبروفين أو غيره من مضادات الالتهاب غير الستيرويدية. إذا كانت لديك أعراض خفيفة، فقد يوصي طبيبك بالتعافي في المنزل. وقد يعطيك تعليمات خاصة لمراقبة الأعراض وتجنب نقل المرض للآخرين. ومن المحتمل أن يُطلب منك عزل نفسك قدر الإمكان عن أفراد أسرتك وعن الحيوانات الأليفة أثناء فترة مرضك، وارتداء كمامة عندما تكون محاطًا بأشخاص وبالقرب من الحيوانات الأليفة، بالإضافة إلى استخدام غرفة نوم وحمام منفصلين. حافظ أيضًا على استمرار تدفق الهواء في منزلك عن طريق فتح النوافذ واستخدام مرشحات الهواء عندما يكون ذلك ممكنًا. وسيطلب منك الطبيب على الأرجح البقاء في العزل المنزلي لفترة من الوقت، مع استثناء الخروج للحصول على الرعاية الطبية. ومن المرجح أن الطبيب سيتابع حالتك بانتظام. وينبغي لك اتباع تعليمات طبيبك وإرشادات إدارة الصحة المحلية بشأن موعد انتهاء العزل المنزلي. وإذا كان مرضك شديدًا، فقد تحتاج إلى الإقامة في المستشفى للعلاج.

من الطبيعي أن نشعر بالخوف والقلق في ظل جائحة فيروس كورونا (كوفيد 19). ومن المرجح أنك قلِق من أن تُصاب أنت أو أحبّتك بالمرض. ولعلّك تحمل همّ الاعتناء بنفسك وبالمرضى. في هذه الأوقات، تذكر الاعتناء بنفسك والتغلب على التوتر. تناول وجبات صحية. احصل على قسط كاف من النوم. مارس الأنشطة البدنية. جرِّب تمارين الاسترخاء مثل التنفُّس العميق وتمارين الإطالة والتأمّل. تجنَّب مشاهدة أو قراءة الكثير من الأخبار. ابق على تواصل مع الأصدقاء والعائلة، عبر الهاتف أو مكالمات الفيديو مثلاً. مارِس الأنشطة التي تستمتع بها، مثل قراءة كتاب أو مشاهدة فيلم كوميدي. إذا كنت مصابًا بفيروس كوفيد 19، فمن المهم أن تلتزم بما يلي: احصل على قسط وافر من الراحة. اشرب السوائل. أبلغ الطبيب على الفور إذا تفاقمت الأعراض. يمكن للإصابة بمرض كوفيد 19 أو الاعتناء بشخص مصاب به أن يسبب التوتر والقلق. فإذا كان التوتر يؤثر في حياتك اليومية لعدة أيام، فعليك الاتصال بالطبيب، وطلب الإحالة إلى اختصاصي صحة نفسية.
